Without the extensions,the heat from the exhaust might melt the shifter cable.
Taller front tire might help.
Longer ramps and a jack under the trailer tongue may help.

What accessories are you running? I only use the water pump and alt.I use 5.0 pullies with a .250 spacer behind the water pump pulley.The water pump pulley can be gotten groved for better grip.

I used the Moroso "race only" kit and wore out the motor shaft bushings in 3 seasons at the track.Worked well untill it didn't.

I had the same problem on my 95 F-350 when towing my trailer.Problem was an old style brake controller,wired into the Ford harness telling the transmission that the brakes were being applied.
